Greetings.
Ensign Jackson has referred me to a software program which allows me to use voice commands with my headset to control certain in-game functions.
Voice Attack is stand-alone, and runs minimized in your task bar.
With Voice Attack, you can create specialized macro voice recognition commands to do certain functions in the game.
These functions are limited to keyboard strokes, to the best of my knowledge. So you won't be able to program it to click any special toolbar box, but you can program it, for example, to "target shields" by moving that command to Ctrl+1 on your expanded toolbar.
Voice Attack has a free 21 day trial period, allowing you to personalize 20 commands for one single profile. After that, you can purchase registration for $8 US. Purchasing the registration allows you infinite personalized macros, and separate macro profiles. For instance, you could arrange to have separate voice commands on other games such as WoW or Lords of the Rings Online, using different profile sets.

Some quick pointers when setting up your macros using Voice Attack:
THE MODIFIER KEYS DON'T WORK
When setting up for a Ctrl+, Shift+, Alt+ type key combination, it works best if you set up a delay time of 0.050 seconds. That's the shortest delay I tested, which still worked.
THE ARROW UP/DN/RT/LT KEYS FOR SHIELD POWER DON'T WORK
We found out that we have to add a second key-binding set for these keys in game. Going to Options, and Key Binds for Space, I set up Shift+A for aft shield, Shift+F for fore shields, Shift+S for Starboard shields, and Shift+P for port shields.
THE PORT SHIELD COMMAND OPENS MY POWER WINDOW
This happened to me. I tested typing shift+p and it doesn't open the window, but when Voice Attack does it, it opens it. L opens your log, so I used T for port.

I too have been fooling with it, and the modifier key workaround I came up with is as follows (in case the other answer for some reason does not work):
Press down MODIFIER key
Pause 0.3 seconds
Press [WHATEVER] key and hold for 0.1 seconds and release
Release MODIFIER key
The other nice thing I have set up is I use the same button to temporarily "turn off" VoiceAttack as I use to "turn on" Vent (a side-button on my mouse) so it "swaps" listening when I want to talk on Vent. Neat little trick so VoiceAttack doesn't do anything wonky when you're trying to have a conversation.

The power window thing is a STO specific macro issue. It occurs when I use macros on my G19, X52, or Naga as well.
Basically, for some reason, if you have a key mapped to a button with modifiers it'll fire off any button with the common keybind.
For example, the default ground commands are (I believe )1 for the first bank, Ctrl + 1 for the second bank, Alt + 1 for the third bank. If I write a macro to fire Ctrl +1 for my second bank's first slot, it will end up firing all three 1s.
I've seen this being discussed on a lot of forums, but so far I've not found any work around that's 100% effective. Some people claim that extending the pauses slightly helps, others say it doesn't, so your mileage may vary.
